Active Lives Pathway Launch Video
The Active Lives Pathway supports people who are physically inactive to attend a community based programme that will encourage and enable them to move more and sit less.
The Active Lives Pathway has 3 Tiers of programmes that your Health and Social Care Professional can refer you (Tiers 2-4). If you are signposted to a Tier 2-4 programme, you will receive a free block of activity lasting either 8 or 16 weeks.
Each Tier has been specially designed to support and enable people to be more physically active. Your Health and Social Care Professional will guide you to the right Tier dependent on your individual needs, confidence levels and motivations.
If you are physically active, your Health and Social Care Professional might signpost you to either a local activity or the regional physical activity website, DG Doing More.
Below you will find information and videos on each Tier of the Active Lives Pathway, so you can find out more before attending. If you are unsure what type of programme you have been referred to, don’t worry a member of the Active Communities will discuss this with you.
