Living Well with a Chest, Heart or Stroke Condition
Supported Self-Management Programme
Living with a long-term health condition can affect your energy, confidence and day-to-day life. This free programme will help you develop practical skills, build confidence and take positive steps towards living well with your condition.
The programme is delivered in partnership between Dumfries & Galloway Health and Social Care Partnership and Chest Heart & Stroke Scotland. It is open to people living with chest, heart and stroke conditions, Long Covid, ME and other energy-limiting conditions
Is this programme for me?
This programme is open to adults living with:
- Chest conditions
- Heart conditions
- Stroke or TIA
- Long Covid
- ME / Chronic Fatigue Syndrome
- Other energy-limiting conditions
What will I gain from taking part?
Over six weekly sessions you’ll explore practical ways to:
- Manage fatigue and energy levels
- Improve sleep
- Build confidence
- Manage stress and anxiety
- Become more physically active
- Set goals that matter to you
You’ll also have the opportunity to connect with others who understand the challenges of living with a long-term condition.
How is it delivered?
- Six weekly group sessions
- Local venues across Dumfries and Galloway
- Online options available
- Delivered by Chest Heart & Stroke Scotland
How do I join?
You can access the programme through:
- Self-referral
- GP referral
- Referral from another health professional
- Referral from a community or voluntary organisation
Self-referrals and professional referrals are welcome.
